We have some basic information from you to initiate services. Please complete a Referral Form to begin.
Referral Form Submission Options
- Online – use our secure online Referral Form or Spanish Secure online Referral Form. You will receive a copy of your completed form automatically when finished.
- Call – 404.378.2300 and press 5 for Referrals. Our referral agent will take basic information. Please have the insurance number, date of birth, and social security number for the referred individual.
We contact all referred individuals within 48 hours of information receipt on Monday through Thursday to schedule an appointment. Submissions made on Friday will be processed on the following Monday. Thank you!
